What is Nucala and How Does It Work?
Nucala (mepolizumab) is a prescription biologic medication used to treat several chronic inflammatory conditions. It is commonly prescribed for severe eosinophilic asthma, chronic rhinosinusitis with nasal polyps, eosinophilic granulomatosis with polyangiitis (EGPA), hypereosinophilic syndrome (HES), and eosinophilic chronic obstructive pulmonary disease (COPD).
The active ingredient, mepolizumab, works as an immunosuppressant. It targets and blocks interleukin-5, a specific protein responsible for the survival and activation of eosinophils. Eosinophils are white blood cells that, in high numbers, cause severe inflammation and tissue damage. By reducing these levels, Nucala helps control swelling and alleviates chronic symptoms.
Dosage and Administration Guidelines
Nucala is administered as a subcutaneous injection under the skin of the thigh, abdomen, or upper arm. It is typically given once every four weeks. The medication comes in three distinct forms: a prefilled syringe, a prefilled auto-injector, and a powder vial that requires reconstitution by a healthcare professional.
While healthcare providers often administer the first doses in a clinic, some patients or caregivers may be trained to perform the injections at home using the prefilled syringe or auto-injector. If a dose is missed, it should be administered as soon as remembered, followed by the regular schedule. Patients should never take a double dose to compensate for a missed one.
Potential Side Effects and Safety Considerations
Like most biologic therapies, Nucala can cause side effects. The most common mild reactions include headaches, injection site pain or redness, fatigue, back pain, and joint discomfort. These symptoms often resolve within a few days or weeks.
Although rare, serious side effects can occur. These include severe allergic reactions and an increased risk of shingles infections. Patients should discuss their medical history with their doctor before starting treatment, especially regarding parasitic infections, vaccination status, and pregnancy.
