Hidradenitis Suppurativa (HS) is a chronic inflammatory skin condition characterized by painful lumps, abscesses, and scarring. While its exact cause remains elusive, the understanding of HS has evolved. Initially considered purely inflammatory, experts now often view it as a follicular occlusion disorder, where hair follicles become blocked and rupture, leading to inflammation.

The Role of Hormones in HS

Emerging research suggests that hormonal dysfunction and fluctuations in estrogen and progesterone levels may play a significant role in some cases of HS. A 2021 review highlighted that the precise mechanisms are still unclear, but several studies indicate a strong correlation between hormonal cycles and symptom severity. For instance, a substantial percentage of women, ranging from 43% to 63%, report a worsening of their HS symptoms around the start of their menstrual periods, typically in the week leading up to them. The reasons behind this cyclical exacerbation are not yet fully understood.

HS and Polycystic Ovary Syndrome (PCOS)

Individuals with HS are also observed to have a higher likelihood of developing Polycystic Ovary Syndrome (PCOS). PCOS itself is a complex endocrine disorder with an unclear etiology, but it is frequently associated with hormonal imbalances, particularly lower progesterone levels and elevated androgen (male sex hormone) levels. This connection further underscores the potential influence of hormones on the manifestation and progression of HS.

Beyond Hormones: Other Contributing Factors

While hormones are an important aspect of HS, it is crucial to recognize that the condition is multifactorial. Other significant factors that can influence the development and severity of HS include genetic predispositions, dysregulation of the immune system, obesity, dietary habits, smoking, and alterations in the skin’s microbiome. A comprehensive understanding of HS requires considering this interplay of various biological and environmental elements.
